IMPORTANT NOTE
Submittal of this form to Sue Edstrom means that I have read and understand the terms for use of the facility & kitchen listed below and agree with these terms.

TERMS OF FACILITY USAGE
1.   All facility usage is by approval only.
2.   Only Bridges ministry events will be considered.
3.   A request of usage must be requested at least 60 days in advance
4.   Rooms are equipped with a variety of items. Those items are to be used according to their design and must not be removed from the room.
5.   Your group may be held accountable for any damaged or missing items.
6.   Use of Bridges multimedia equipment for the event may require training prior to event, upon discretion of BCC staff.

 

TERMS OF KITCHEN USAGE
1.   All kitchen usage is by permission or notification only.
2.   All items left in the kitchen or temporary storage must be labeled by (group) name and date.
3.   All items not labeled or dated over two weeks will be discarded.
4.   Follow the directions posted in the kitchen for use of the garbage disposal, sink and countertops.
5.   Facillities personnel must supervise the initial operation of the gas oven and/or stove/grill.
6.   Use of electric or gas appliances must be by a knowledgeable or trained individual at least 18 years of age.
7.   All areas of kitchen must be cleared away of items, cleaned up and wiped following your acitivity.
8.   Do not use any supplies in the kitchen unless they are labeled specifically for "Community Use."
9.   Secure the kitchen area, making sure all doors are locked & keys returned to the staff office, prior to leaving.
10. If you have any questions or if community supplies were depleted signnificantly (e.g. no more towels)  contact Sue Edstrom @ 651-2030x204 or
